The name on the form must EXACTLY match your degree certificate and passport. Even minor differences (e.g., "Vivek Sharma" vs "Vivek Kumar Sharma") cause issues. Use your full official name as on the degree certificate.
Mistake 2: Wrong Registration Number
Some universities have multiple ID systems (registration number, enrollment number, roll number). Provide the specific identifier the university uses for record retrieval.

For international applications, the destination authority's reference number must be provided. Without it, the university doesn't know how to label the dispatched envelope.

For sealed dispatch, the complete destination address must be filled (not just "WES Toronto"). Include full street, city, state, ZIP/postal code, country.
